I get paper checks from BookMaker; they charge $65 per check. The website used to say that free payouts were only with paper check, now they say only with debit card. I should ask them about that over the phone, but I haven't.

I'm also amused to see that my checks are increasingly sketchy. The last one was from a "Mr. Pooloo" drawn on (seriously) the Bank of Mauritius.
